Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care in the comfort of their own homes. Our team is dedicated to delivering exceptional care and making a positive impact on the lives of our patients and their families.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and address any concerns or questions.
  • Document patient care and progress in a timely and accurate manner, adhering to regulatory requirements and company policies.
Requirements
  • Graduate of an accredited Licensed Practical Nurse program.
  • Current LPN licensure in the state of Washington.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift, push, and pull up to 25 pounds.
Preferred Qualifications
  • 1 year of recent hospital, nursing home, or home health experience.
