LPN - Home Health Care Specialist

Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in Spokane, WA. As an LPN, you will work alongside an interdisciplinary team to provide high-quality patient care in the comfort of their own homes.
Responsibilities:
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the healthcare team to ensure seamless care transitions and coordination.
- Communicate effectively with patients, families, and healthcare providers to ensure optimal care outcomes.
Requirements:
- Graduate of an accredited Licensed Practical/Vocational Nurse program.
- Current Washington State LPN licensure.
- Basic Life Support (BLS) certification.
- Ability to obtain an NPI number within 30 days of hire.
Preferred Qualifications:
- 1 year of recent hospital, nursing home, or home health experience.
