Capstone Hospice is seeking a dedicated and compassionate Registered Nurse to join our team as a Case Manager. This is a full-time position with a regular schedule of Monday to Friday, 8am to 5pm.
Job Summary:The RN Case Manager will be responsible for managing patient care for a designated caseload of patients residing in their personal homes and assisted living facilities. This role requires strong communication and interpersonal skills, as well as the ability to work independently and make sound judgments.
Key Responsibilities:- Observe and report any changes in patient condition to the physician and other team members.
- Teach and counsel patients and their families on providing care for the patient.
- Develop and re-evaluate patient care plans in conjunction with the Interdisciplinary Group (IDG) to meet patient needs and maintain continuity of care.
- Document visits in the electronic medical record system (Wellsky Consolo).
- Perform specific nursing procedures as needed, following doctor's orders.
- Graduate from an accredited school of nursing.
- Currently licensed as a registered nurse to practice in the state of Georgia.
- Minimum of 2 years as a registered nurse.
- Active patient contact within the past three years, preferred.
