Job Summary:

The RN Case Manager will be responsible for managing patient care for a designated caseload of patients living in their personal homes and assisted living facilities. This role requires strong communication and interpersonal skills, as well as the ability to work independently and make accurate judgments in a fast-paced environment.

Key Responsibilities:
  • Observe signs and symptoms, and report any unexpected changes to the physician and other team members.
  • Teach, supervise, and counsel patients and family members on providing care for the patient.
  • Develop and re-evaluate patient/family care plans in conjunction with the Interdisciplinary Group (IDG) to meet needs and maintain continuity of care.
  • Document visits in the electronic medical record system (Wellsky Consolo).
  • Perform specific nursing procedures as needed, following doctor's orders.
Requirements:
  • Graduate from an accredited school of nursing.
  • Currently licensed as a registered nurse to practice in the state of Georgia.
  • Minimum of 2 years as a registered nurse.
  • Active patient contact within the past three years, preferred.
Working Conditions:

This position requires the ability to tolerate exposure to elements such as odors, blood, body fluids, excrements, adverse environmental conditions, and hazardous materials.
