Clinical Nurse Navigator RN Role

About the Role:
This is an exciting opportunity for a skilled Clinical Nurse Navigator RN to join our gastroenterology team at Rush University Medical Center. As a key member of our interdisciplinary team, you will play a vital role in ensuring seamless transitions between hospital and community settings, promoting evidence-based practices, and enhancing patient outcomes.
Key Responsibilities:
• Provide expert consultation in specialty areas to all levels of healthcare providers
• Offer emotional support and guidance to patients regarding follow-up and treatment plans
• Arrange appointments, review test preparation procedures, and assess potential patient barriers
• Contact patients via phone calls, emails, and letters to remind them about appointments and answer questions about testing and treatment
• Perform follow-up phone calls to patients after appointments as indicated
• Develop mechanisms for sites to share patient data in a manner that optimizes their time and effort
• Complete electronic records and paperwork regarding patient interactions and appointments
