About The Job

The Jewish Board is seeking a skilled and compassionate Caregiver Support Specialist - Intellectual Disabilities to join our dedicated team. As a Caregiver Support Specialist, you will play a vital role in supporting individuals with intellectual disabilities in our Home and Community Based Setting (HCBS) and Day Habilitation programs.

About the Position

This position involves providing personalized support and care to individuals with intellectual disabilities, promoting their independence, autonomy, and overall well-being. You will work closely with clients, their guardians/advocates, and supervisors to identify valued outcomes and develop individualized habilitation plans.

Your Key Responsibilities

  • Provide direct care and support to individuals with intellectual disabilities in their homes or day habilitation settings
  • Assist with personal hygiene needs, daily living tasks, and other activities as needed
  • Develop and implement activities that promote independence, socialization, and community integration
  • Document client progress, maintain accurate records, and communicate effectively with the treatment team
  • Participate in interdisciplinary team meetings to review client progress and plan for future goals
