Position Objective:

  • Facilitate access to comprehensive health care services for eligible patients, particularly pregnant women and children aged 0-5 years.
  • Emphasize intensive management of care services tailored for specific demographic groups.

Required Education and Experience:

  • Bachelor's degree in social work or a related human services discipline.
  • Preferred: Two years of relevant experience.

Licensure or Certification: Must possess a valid NC driver's license.

Key Responsibilities:

  • Collaborate with healthcare providers and organizations to develop patient-centered care strategies.
  • Execute care management processes aimed at enhancing patient health outcomes.
  • Evaluate patients to determine the appropriate level of care management services needed.
  • Build and sustain relationships with medical homes and healthcare providers.
  • Oversee crisis prevention and stabilization efforts for patients.
  • Provide education to patients regarding self-management techniques and available resources.
  • Assess and prioritize patient referrals effectively.
  • Maintain a caseload in alignment with program expectations.
  • Monitor care management metrics to evaluate progress.
