Job Summary

The Licensed Clinical Social Worker is a critical member of our team, responsible for providing initial evaluations and individual and group therapy to patients. This role requires a strong clinical knowledge base, excellent communication and interpersonal skills, and the ability to work independently.

Key Responsibilities
  • Conduct initial evaluations and provide individual and group therapy to patients
  • Collaborate with physicians and other patient care team members to ensure comprehensive care
  • Provide care coordination and management to patients with social determinants of health challenges
  • Ensure that medical record documentation protocols are complied with
  • Provide in-home patient visits as needed
Requirements
  • Master's degree in social work
  • Current licensure as a Licensed Clinical Social Worker with at least three years of behavioral healthcare experience
  • Experience in a primary healthcare setting is preferred
  • Bilingual in Spanish is preferred
