Job Summary

We are seeking a skilled Licensed Social Worker to join our care management team at the University of Arkansas System. As a member of our team, you will play a critical role in assisting patients and their families navigate the healthcare system, providing emotional support, and coordinating care to ensure the best possible outcomes.

Key Responsibilities
  • Assess patients' psychological, financial, and discharge planning needs
  • Provide information, support, counseling, and referrals to other levels of care and resources
  • Collaborate with the multi-disciplinary team to develop and implement patient-centered care plans
  • Document patient choices and preferences in the medical record
  • Participate in hospital-wide performance improvement metrics and regulatory compliance initiatives
Requirements
  • Masters' degree in Social Work
  • Licensed by an Arkansas Professional Board as a Licensed Master Social Worker (LMSW) or Licensed Certified Social Worker (LCSW)
  • 2 years of experience in an Acute Care Hospital Setting (preferred)
