The Opportunity

We're seeking a highly skilled and compassionate Social Worker to join our High Risk Care team. As a key member of our interdisciplinary team, you'll work closely with patients, caregivers, and healthcare providers to provide comprehensive, coordinated care.

Your primary responsibilities will include assessing and developing care plans that address biopsychosocial barriers to patients' goals. You'll also provide complex case management, resource navigation, and advocacy to ensure patients receive the support they need to thrive.

Key Responsibilities
  • Develop and implement comprehensive care plans that address patients' biopsychosocial needs
  • Provide complex case management and resource navigation to patients and caregivers
  • Advocate for patients' rights and interests, ensuring they receive equitable care
  • Collaborate with healthcare providers to coordinate care and address patient needs
  • Facilitate discussions around advanced care planning, long-term care planning, and palliative care
Requirements
  • Licensed Master of Social Work (LMSW) required, with ability to achieve reciprocity to cover additional state markets within a year of employment
  • 3+ years of experience as a Licensed Social Worker in high-risk, complex care settings, senior health, and/or case management
  • Master of Social Work (MSW) required
  • Experience with home-based care services, hospitals/SNF, and long-term care facilities preferred
