Job Summary

We are seeking a highly skilled and compassionate High Risk Case Manager to join our team. As a High Risk Case Manager, you will work closely with patients, families, and healthcare providers to develop and implement comprehensive care plans that address the biopsychosocial needs of our patients.

Key Responsibilities

  • Conduct thorough biopsychosocial assessments to identify patients' needs and develop personalized care plans
  • Collaborate with patients, families, and healthcare providers to implement care plans and address barriers to care
  • Provide education and support to patients and families to promote self-management and empowerment
  • Link patients to community resources and services to address social determinants of health
  • Facilitate transitions of care and ensure seamless communication between healthcare providers

Requirements

  • Licensed Master of Social Work (LMSW) required with ability to achieve reciprocity to cover additional state markets within a year of employment
  • 3+ years of experience as a Licensed Social Worker with demonstrated experience in high risk, complex care settings, senior health, and/or case management experience
  • Master of Social Work (MSW) required
  • Experience with home based care services, hospitals/ SNF and long term care facilities preferred
  • Demonstrated skill in biopsychosocial assessments, resource navigation, care plan development, and coordination across healthcare settings on behalf of very complex patient needs
