LPN Job Description

Klarus Home Health is seeking a part-time Licensed Practical Nurse (LPN) to provide community-based care for our patients in the Santa Fe, NM area.

Job Responsibilities
  • Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
  • Coordinate care with the interdisciplinary group, patient/family, and referring agency.
  • Assist in the development and implementation of patient care plans.
  • Monitor patient health and report any changes to the supervising RN.
  • Administer medications and treatments as prescribed.
  • Educate patients and their families on health care needs, conditions, and options.
  • Maintain accurate and timely documentation of patient care and progress.
Requirements
  • Graduate of an accredited School of Vocational/Practical Nursing.
  • Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC).
  • Must have a current driver's license.
  • Two years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.
