About the Role: The Social Services Director will be responsible for monitoring the mental and psychosocial needs of our residents and providing services to meet these needs, ensuring the highest practical level of physical, mental, and psychosocial well-being.

Key Responsibilities:

  • Develop and Implement Care Plans: Develop an ongoing care plan based on the residents' needs, interests, and abilities as identified in the initial assessment.
  • Participate in Care Plan Meetings: Participate in care plan meetings to ensure the best possible outcomes for our residents.
  • Record Keeping and Documentation: Complete required record keeping upon resident admission, discharge, transfer; chart and document per facility policy.
  • Healthcare Directives and End of Life Care: Implement healthcare directives and identify end of life care opportunities.
  • Discharge Planning: Assess the resident's potential for discharge as a member of an interdisciplinary team, plan for the resident's discharge and post-discharge with the resident, family, and interdisciplinary team, and assist the resident and family in the implementation of the discharge plan.

Requirements:

  • Education: Bachelor's degree in social work.
  • Experience: 2-3 years' experience in social services/discharge planning in a long-term care/Skilled nursing setting.
  • Knowledge: Working knowledge of Medicare/Medicaid & Insurance.
