As a Licensed Practical Nurse (LPN) under the supervision of an RN Case Manager, you will be responsible for visiting patients in their residences or care facilities. You will adhere to each patient's individualized care plan, delivering direct nursing care while meticulously documenting each visit using the Netsmart EMR system. It is essential to communicate any changes in the patient's health status to the RN Case Manager promptly.
This is a Full-Time overnight position.
- Must be a graduate of an accredited Licensed Practical Nurse Program.
- A minimum of one year of experience as an LPN in medical-surgical nursing is required, along with at least one year of experience in hospice or oncology nursing.
- Proficient computer skills are essential.
- A valid driver's license and reliable transportation are necessary for patient visits.
