Job Title: Care Transition Specialist, Lead

Mass General Brigham is seeking a highly skilled and experienced Care Transition Specialist to lead our team in providing exceptional patient care and coordination.

Job Summary:

The Care Transition Specialist, Lead will be responsible for managing the care transition process for patients, ensuring seamless transitions between healthcare settings, and providing education and support to patients and families. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Manage the care transition process for patients, including coordinating with healthcare providers, facilities, and community resources.
  • Provide education and support to patients and families, ensuring they understand their care plan and are prepared for discharge.
  • Collaborate with the care team to identify and address any barriers to care, ensuring patients receive the necessary resources and services.
  • Develop and maintain relationships with healthcare providers, facilities, and community resources to ensure smooth transitions and optimal patient outcomes.
  • Monitor and evaluate the effectiveness of care transitions, identifying areas for improvement and implementing changes as needed.
Requirements:
  • Bachelor's degree in a related field, such as nursing, healthcare administration, or social work.
  • 3-5 years of experience in healthcare, preferably in care coordination, case management, or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work in a fast-paced environment, with strong organizational and time management skills.
  • Knowledge of healthcare regulations, policies, and procedures, with the ability to apply this knowledge in a practical setting.
