About the Role:

The Memory Care Director will oversee the management and performance of the Memory Care neighborhood, ensuring the highest quality of resident care and customer service. This position will be responsible for meeting the psycho/social needs of residents, conducting pre-residency assessments, and evaluating the psychosocial and activity of daily living (ADL) needs of residents in Memory Care.

Key Responsibilities:

  • Develop and implement a holistic, Person-Centered approach to assessment and care planning
  • Partner with other departments to develop support groups and educational programming opportunities for residents and family members
  • Support Independent Living and Post-Acute Social Workers to effectively transition residents between levels of care
  • Maintain a working knowledge and ensures compliance of Federal, State, and local regulations, as well as facility policies regarding Memory Care

Requirements:

  • A 4-year degree in a related field, such as Social Work, Nursing, or Gerontology
  • A minimum of 3 years of related experience providing services for residents with cognitive impairment and/or leading a Memory Care or Assisted Living neighborhood
  • Knowledge of the geriatric population and the aging process, including the physical, psychological, and social needs of the elderly
