Overview

As a Patient Advocacy Manager, you will play a key role in advocating for our patients' needs and preferences. You will work closely with patients, their families, and healthcare providers to ensure that patients receive high-quality care that meets their individual needs.

Responsibilities
  • Advocate for patients' needs and preferences, ensuring that their voices are heard and respected by healthcare providers.
  • Develop and implement strategies to improve patient satisfaction and engagement, including patient education and support.
  • Collaborate with healthcare providers to ensure that patients receive coordinated and comprehensive care that addresses their physical, emotional, and social needs.
  • Evaluate the effectiveness of patient advocacy efforts and make necessary adjustments to optimize patient outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
Requirements

To be successful in this role, you will need:

  • Experience with patient advocacy and care coordination.
  • Two (2) years' experience in a healthcare setting.
  • Excellent communication and interpersonal skills.
  • Proficiency with Microsoft Office, computer skills.
  • Current LPN License.
  • One (1) – Two (2) years' experience in case management (preferred).
  • Current RN License (preferred).

**Estimated Salary:** $70,000 - $90,000 per year, depending on experience.
