Job Summary

A Licensed Practical Nurse (LPN) working in our Skilled Nursing facility is responsible for providing high-quality nursing care to our residents. This position is a valuable clinical team member contributing to the smooth, organized, and efficient operations of the nursing department.

Responsibilities
  • Provide primary nursing care to residents with varying backgrounds and medical histories.
  • Assist with new admissions and monitor resident conditions.
  • Communicate changes of condition to healthcare providers and implement medical interventions as needed.
  • Communicate resident needs to outside providers, including primary care physicians, home health agencies, and pharmacies.
  • Implement and follow healthcare providers' orders and communicate resident conditions to family members and responsible parties.
  • Perform other duties as assigned.
Requirements

To be successful in this role, you must have:

  • Licensed Practical Nurse (LPN) licensure in the state of practice.
