The Licensed Practical Nurse works within the healthcare team and provides focused community and clinic health services directed towards the prevention of disease, the promotion of health, improving clients self
-care, using professional judgment, screening, contributing to care planning, intervention, and provides health information/education to patients.

In the Licensed Community Practical Nurse role you will:

  • Utilizing electronic reports and review provider schedules to proactively assess needs for prevention screening and communicating via EHR.
  • Proactively performs previsit activities (e.g. scrubbing and huddling) for client visits as needed, As well as preparing patients for exams, and procedures, including equipment setup and specimen collection.
  • Working with providers and teams to monitor chronic disease and prevention reports and initiates followup with clients or ensures appropriate referrals as needed.
  • Following up on specialty referral reports, clinical treatments, prescriptions, medication cancellations, etc.
  • Documenting clearly in the patient chart, using the SOAP method, if indicated, and EPIC "dotphrases" and "smart phrases" as deemed appropriate per patient encounter.
  • Supporting back office clinic flow by direct provider support as needed including rooming patients, ordering labs as directed.
  • Providing after visit instructions based on nursing experience and understanding of client needs; makes followup appointments, if necessary.
  • Administering medical treatments such as cleaning and dressing wounds, ear irrigations, suture removal, catheterizations, fluoride treatments, etc. Performs diagnostic tests such as labs, phlebotomy, EKG, etc.
  • Work with RN and PCP to contribute to the plan of care.
  • Completing documentation and charting as required by policy.
  • Troubleshooting equipment issues and reporting necessary repairs as needed.
  • Completing assigned Quality Control/Assurance activities.

Minimum Qualifications/Transferable Skills*:

  • An Associate's or Bachelor's degree in nursing from an accredited nursing program
  • Possession of, or ability to obtain, an appropriate and valid license to practice as a Licensed Practical Nurse issued by the Oregon State Board of Nursing at the time of hire
  • A minimum one (1) year of nursing experience
  • Valid CPR/BLS certification upon hire
