Job Summary

We are seeking a highly skilled and compassionate Patient Services Coordinator to join our team. As a Patient Services Coordinator, you will play a critical role in ensuring the timely scheduling of patient assessment visits and the efficient assignment of clinicians.

Key Responsibilities:
  • Ensure patient assessment visits, including all Oasis visits, are scheduled and performed in a timely manner.
  • Work collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center.
  • Manage calls from patients and field staff related to scheduling issues.
  • Refers clinical and performance-related issues to the clinical manager.
  • Monitor hospitalized patients, communicating and ensuring the team follows up as necessary.
  • Support clinical manager and work collaboratively to ensure timely communication with patients, clinicians, referral sources, business development team, and other office personnel.
  • Run all applicable reports and respond to workflow, taking appropriate actions.
  • Assist with internal or external transfer of patients between care centers and/or hospice services.
  • May be responsible for maintaining supply closet with routine supplies, ensuring supplies are within expiration dates and packaged appropriately, and serving as liaison with the field for patient supply needs.
Requirements:
  • High School diploma or GED equivalent.
  • Preferred: One year of scheduling experience in a healthcare environment or one year of administrative experience in a healthcare environment.
