Key Responsibilities
  • Referral Management: Receive and analyze incoming referrals and orders, ensuring accurate and timely processing.
  • Communication: Effectively communicate patient and referral information to our Intake Team and other stakeholders, ensuring seamless coordination and collaboration.
  • Information Gathering: Obtain demographic and clinical information from clients, physician's offices, and other referral sources to compile referrals.
  • Log Management: Maintain a call log for all incoming calls, ensuring accurate and up-to-date records.
  • Referral Verification: Verify new referring physician licenses on referrals taken in department, ensuring compliance with regulatory requirements.
  • Authorization and Verification: Research coverage or pay source for each referral, verifying commercial policies and HIQA, and obtaining authorization for visits from insurance or Medicaid as appropriate.
  • Patient Education: Educate prospective patients, families, physicians, and hospitals regarding home health services available, ensuring informed decision-making.
  • Liaison Role: Serve as a liaison between clinical and marketing staff, ensuring effective communication and collaboration.
  • Training and Development: Attend and participate in annual in-service training programs, ensuring ongoing professional development and growth.
  • Patient Ping Management: Manage patient ping feed throughout the day, disseminating patient ping data to appropriate Patient Care Manager, field clinician(s), or sales representative as needed.
Requirements
  • Current LPN Licensure: Current LPN licensure in state of practice.
  • Driver's License and Vehicle: Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification: CPR Certification required.
  • Experience: Minimum of one (1) year experience in medical office procedures/intake.
