Job Title: Primary Care Nurse Practitioner

We are seeking an experienced Nurse Practitioner to provide high-quality primary care to homebound and palliative care patients in the Greater Philadelphia region.

Key Responsibilities:
  • Provide comprehensive primary care to patients in their homes, including assessing their health, creating treatment plans, and prescribing medications as needed.
  • Collaborate with our clinical team to ensure seamless care coordination and effective communication with patients, families, and caregivers.
  • Develop and maintain a panel of patients, providing regular check-ins and care planning to ensure optimal health outcomes.
  • Participate in quarterly medical staff meetings to stay up-to-date on best practices and share knowledge with colleagues.
Requirements:
  • Board Certified Nurse Practitioner with a valid NP certification in the state of Pennsylvania.
  • Strong clinical skills and ability to accurately diagnose and treat patients.
  • Positive demeanor and strong people skills, with the ability to build trust with patients and families.
  • Ability to work independently and problem-solve effectively in a fast-paced environment.

  • Competitive salary range: $105,000 - $145,000+ (depending on experience and other incentives).
