Position Summary:
We are seeking a skilled Nurse Practitioner to provide primary and palliative care to homebound patients in the Greater Philadelphia area. In this role, you will utilize your medical expertise under the guidance of a supervising physician, serving a diverse patient population with chronic health issues.
Key Responsibilities:
- Deliver outstanding care to primary and palliative patients in the Greater Philadelphia vicinity.
- Perform comprehensive health evaluations, maintain updated medical records, and develop effective care strategies.
- Engage in clear communication with office personnel and clinical teams through internal communication channels.
- Provide patient-centered care to a designated group of individuals.
- Participate in regular medical staff meetings to discuss patient care.
- Formulate diagnoses, assessments, and treatment plans.
- Build relationships and consult with patients, families, and caregivers.
- Order necessary diagnostic tests and prescribe medications as needed.
Qualifications:
- Board Certified Nurse Practitioner with valid certification in Pennsylvania.
- Proven clinical expertise in assessment, treatment, and diagnosis.
- Positive demeanor and strong interpersonal abilities.
- Ability to solve problems independently and work autonomously.
- 1-2 years of experience in primary care, particularly with geriatric patients (hospice experience preferred).
