Job SummaryWe are seeking an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Greater Philadelphia region.
You will work under the supervision of a physician, applying medical techniques and principles to care for a diverse set of patients with chronic diseases.
Key Responsibilities- Provide high-quality care to primary and palliative patients in and around Greater Philadelphia (Montgomery and Chester County, PA)
- Assess patient health by performing physical examinations, obtaining and updating medical histories, and creating personalized care plans
- Effectively communicate with office staff and clinical operations across various internal platforms
- Provide patient-centered care to a panel of patients
- Attend quarterly medical staff meetings
- Develop impressions, diagnoses, and treatment plans
- Foster trust and consult with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary
- Prescribe medications, supplies, and appropriate interventions
We are looking for a Board Certified Nurse Practitioner with a valid NP certification in the state of Pennsylvania.
Strong clinical skills, a positive demeanor, and strong people skills are essential.
Ability to problem-solve and operate independently, with 1-2 years of primary care experience and geriatric patient care (hospice experience highly preferred but not required).
