We are seeking a dedicated and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring that our patients receive the highest level of service possible.

The successful candidate will have excellent communication and organizational skills, as well as the ability to work collaboratively with other healthcare professionals to achieve optimal patient outcomes.

Key Responsibilities:

• Coordinate patient care by communicating with patients, families, and healthcare providers to ensure that all aspects of care are addressed and met.
• Develop and implement patient care plans, taking into account the unique needs and circumstances of each patient.
• Collaborate with members of the healthcare team to ensure seamless communication and collaboration.
• Monitor patient condition and report any changes to the healthcare team.
• Maintain accurate and detailed records of patient interactions and care provided.

Requirements:

• Licensure/Certification/Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Registered Nurse credentialed from the Texas Board of Nursing required. Advanced Life Support preferred. American Heart Association or American Red Cross accepted. Pediatric Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
• Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
• Experience: Minimum of 2 years experience in field of specialty, in an Acute Hospital required.

Estimated Salary Range:$60,500 - $88,000 per year
