Nurse Care Coordinator

As a Nurse Care Coordinator with ArchCare, you will play a vital role in enhancing the lives of our elders and others who need extra help to stay healthy and live life to its fullest. This is a privilege and our calling.
Key Responsibilities:
• Assess participants' physical and functional needs, along with psychosocial status and needs, home safety, family caregiver burden, etc.
• Develop and implement the home care components and work with other team members to develop a comprehensive Interdisciplinary team plan of care based on the needs of participant and caregivers.
• Monitor effectiveness and outcomes regularly and keep the team informed as to participant progress and level of need.
Requirements:
• Current NY State Registered Nurse License
• Valid Drivers License and Transportation for home visits
