Job Title: Licensed Practical Nurse - Community Care Specialist

BAMSI is a nonprofit organization dedicated to empowering individuals with developmental disabilities and mental health challenges. We are seeking a Licensed Practical Nurse to join our team as a Community Care Specialist.

Job Summary: The Licensed Practical Nurse will provide high-quality nursing care to individuals with complex medical needs, developmental disabilities, acquired brain injury, and/or mental health issues in a community-based setting.

Responsibilities:

  • Administer medications and treatments following established protocols and policies.
  • Conduct daily written notes, data tracking, and skilled assessments to ensure the health and well-being of individuals served.
  • Communicate health changes in a timely manner to ensure seamless care coordination.
  • Utilize Positive Behavior Supports (PBS) practices to create a supportive work environment that promotes growth, dignity, and independence.
  • Provide physical support, assistance with lifting and transferring individuals served as needed.
  • Collaborate with the direct care and supervisory team to ensure the health status of individuals served.
  • Provide oversight of meals, train staff on meal preparations, and ensure food and meals are prepared properly.
  • Assist with the maintenance of medication books, house log books, and other records to ensure program compliance.
  • Follow admission and discharge requirements for hospitalized individuals.
  • Provide training to staff and individuals served in relevant areas, such as health, hygiene, safety, and diet.
  • Provide holiday coverage on a rotating basis and attend staff meetings.
  • Ensure safety standards and infection control protocols are followed.

Requirements:

  • Possession of a Massachusetts license as a nurse.
  • Valid driver's license in the state of residence and 3 years of driving experience.
  • Basic computer literacy, including the ability to record data, use electronic time and attendance software, and receive and send email.
