Overview of the Role

We are seeking a skilled Clinical Health Specialist to join our team in Buffalo, NY. In this role, you will conduct comprehensive health assessments in patients' homes, leveraging your expertise to identify and address potential health risks.

Responsibilities

  • Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
  • Collaborate with Primary Care Physician (PCP) on patient education and provide follow-up support.
  • Provide services in various venues, including Home Visits, Skilled Facility Visits, and Telehealth Visits.

What We Offer

As a Clinical Health Specialist, you can expect competitive compensation ranging from $2,300 to $11,500 per month (based on an average 20-100 completed visits per month). Requirements

  • A Master's Degree or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • A minimum of 1 year experience as a Nurse Practitioner/Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.

Travel Requirements

You will be required to travel to meet patients in their homes, possessing a valid state driver's license, proof of adequate automobile insurance coverage, and the ability to drive a car comfortably.
