About the Position

This role involves conducting Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols. You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.

You will provide services in various venues, including Home Visits, Skilled Facility Visits, and Telehealth Visits. Responsibilities
  • Conduct assessments, collect medical history, diagnose and treat patients, educate patients on health, refer to physicians and case managers, follow up on patient progress, and document all interactions according to Matrix protocols.
  • Collaborate with healthcare professionals, including physicians, case managers, and social workers, to ensure comprehensive care for patients.
Requirements
  • Masters Degree required OR commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority.
  • Board certified by NCCPA, AANP, ANCC, or AACN in a Matrix approved specialty.
  • 1 year experience as a Nurse Practitioner/Physician Assistant, with the ability to travel within the state or out of state.
  • Strong computer skills and familiarity with EMR software.
Benefits

The estimated salary for this position is $6,600 - $10,900 per month, depending on the number of completed assessments. Mileage reimbursement is also provided. As a non-benefited PRN provider, you will not receive additional benefits beyond these provisions.
