**Job Description:**

The RN Case Manager will assume primary responsibility for patient caseloads, including assessment, planning, implementation, and evaluation phases of the nursing process. The successful candidate will obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact their needs. They will initiate communication with attending physicians, interdisciplinary teams, and other agencies to coordinate optimal care.

**Responsibilities:**

  1. Provide regular visits to reassess patient needs and update plan of care.
  2. Perform clinical pronouncement of death and provide support to family members.
  3. Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely.
  4. Supervise and document delivery of care by Home Health Aides as required by Federal, State, and Company guidelines.

**Qualifications:**

  • Graduate from an accredited registered nursing program.
  • Licensed as a registered nurse in the state of practice.
  • Current CPR and First Aid Certification.
  • Knowledgeable of quality assessment and assurance procedures.
  • Valid driver's license and proof of insurance required.
