Job Description: We are seeking a skilled Chronic Care Management Specialist to join our team. As a hybrid role, the incumbent will primarily focus on providing Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) services to patients with multiple co-morbidities or specific chronic conditions such as diabetes, cardiovascular disease, and COPD.
Key Responsibilities:
- Works diligently on behalf of assigned patients, ensuring their needs are met with empathy and efficiency.
- Provides Chronic Care Management and Remote Patient Monitoring services, maintaining a comprehensive understanding of program requirements and benefits.
- Gathers accurate information and documents patient interactions in the Electronic Medical Record (EMR) system.
Required Skills and Qualifications: Certification as a Nursing Assistant (CNA), Medical Assistant (MA), or MedTech; strong organizational skills with meticulous attention to detail; excellent communication and interpersonal skills; ability to work independently and collaboratively within a team; proficiency in using Electronic Medical Record (EMR) systems and other relevant software applications.
