Aveanna Healthcare seeks a dedicated and compassionate Registered Nurse to join our team as a Skilled Nursing Care Professional. As a vital member of our healthcare staff, you will be responsible for delivering high-quality patient care in the comfort of patients' homes.
Estimated Salary: $85,000 - $110,000 per year
About the Role:
In this rewarding role, you will work closely with patients, families, and healthcare teams to provide personalized care and support. Your primary responsibilities will include:
- Developing and implementing individualized care plans tailored to each patient's needs
- Providing skilled nursing services, including wound care, medication management, and vital sign monitoring
- Conducting thorough assessments and documenting patient progress
- Collaborating with physicians and other healthcare professionals to ensure seamless care transitions
- Education and counseling patients and their families on disease management, self-care techniques, and prevention strategies
Requirements:
To be successful in this role, you must possess:
- A valid RN license in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
- Excellent communication, problem-solving, and time management skills
Preferred Qualifications:
