Job Overview

POSITION TITLE: Community Living Supervisor

DIVISION: Community Living

SUPERVISOR: Assistant Director, Community Living

CLASSIFICATION: Non-Exempt

OBJECTIVE:

The primary goal of this role is to oversee direct care personnel and manage the daily operations and upkeep of the group home environment. This position is essential in ensuring effective communication within the Agency and the Community Living Services division. The ultimate aim is to create a living space that closely resembles a typical home while promoting the highest level of independence for each client.

KEY RESPONSIBILITIES:
  1. Supervise direct care staff to guarantee the Individualized Program Plan (IPP) is effectively executed for each client.
  2. Provide training to staff across all programs utilizing various educational methods.
  3. Coordinate the provision of training and rehabilitation services.
  4. Monitor the execution and follow-up of each client's Individualized Program Plan.
  5. Organize and lead interdisciplinary staff meetings as necessary for program planning and evaluation.
  6. Oversee documentation and record-keeping processes.
  7. Engage directly in recreational, therapeutic, and training activities with clients.
  8. Act as a liaison among clients, families, staff, professionals, and the community.
  9. Protect the human and civil rights of clients and their families.
  10. Manage medication supplies to ensure adequacy.
  11. Assist clients in managing their finances.
  12. Ensure a sufficient supply of household items is maintained.
  13. Accurately manage petty cash transactions.
  14. Provide both formal and informal assistance to clients with their programming needs.
  15. Deliver services in accordance with State Board of Health regulations.
  16. Encourage client participation in community events.
  17. Exhibit patience in all interactions involving clients, staff, parents, and community members.
  18. Meet the physical demands associated with client care, including lifting and assisting clients as needed.
  19. Ensure compliance with all departmental and agency policies as well as state and federal regulations.
  20. Respond to client-related issues as they arise.
  21. Maintain a safe and secure environment for all clients.
  22. Utilize personal or company devices for necessary communication and time entry.
  23. Ensure all medical needs of clients are met, including transportation to appointments and communication with nursing staff.
ESSENTIAL SKILLS:
  1. Implement all programs promptly and efficiently.
  2. Facilitate staff meetings as required.
  3. Document and provide input for monthly provider/residential meetings.
  4. Route receipts to the Administrative Assistant for processing.
  5. Replenish household supplies as necessary.
  6. Support direct care staff in their roles.
  7. Provide direct care for a minimum of 20 hours per week.
  8. Document personnel issues and communicate them to the QIDP or Director of Resources.
  9. Complete tasks assigned by supervisors.
  10. Accept guidance from supervisors and other management personnel.
  11. Maintain confidentiality regarding all client and staff information.
  12. Coordinate with the business office to ensure timely payment of liabilities.
QUALIFICATIONS:
  1. Bachelor's degree in a human services field is preferred; a High School Diploma or equivalent is required.
  2. A minimum of one year of experience working with the MR/DD population, ideally in a residential setting.
  3. Ability to obtain and maintain certifications in CPR, First Aid, NCI, and Medication Administration.
  4. Strong oral and written communication skills, with a commitment to confidentiality.
  5. Possession of a valid driver's license and appropriate vehicle insurance.
