Job Summary
We are seeking a skilled and compassionate Nurse Care Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support to our residents. This is an exciting opportunity to work in a dynamic and fast-paced environment, where you will have the chance to make a real difference in the lives of our residents.
Key Responsibilities
- Deliver high-quality patient care in accordance with facility clinical systems, regulatory requirements, and physician orders.
- Oversee the direction of Nursing Assistants to ensure that the highest quality services are delivered in a resident-centered environment.
- Assess resident needs and assist in developing and following individual care plans.
- Identify and communicate changes in resident status to physicians, NP, staff, and family members.
- Administer medications, treatments, and other direct care as prescribed and indicated.
- Coordinate admissions, transfers, and discharges of residents.
- Communicate information to staff to provide a smooth shift transition.
- Document appropriately as needed or required in PCC.
Requirements
- Current and unrestricted IL LPN/RN License
- CPR certification
- Excellent verbal and non-verbal communication skills
- Skilled nursing facility experience preferred
