Key Responsibilities:
- Conduct comprehensive reviews and coordinate data within medical records, accurately coding principal and secondary diagnoses and procedures to ensure optimal reimbursement.
- Abstract coded data as mandated by relevant health information policies.
- Engage with attending physicians or physician liaisons to clarify any uncertainties in medical record documentation.
- Extract coded data for evaluation, research, and planning purposes, generating reports as needed.
- Assist in correcting SPARCS data errors as required.
- Support physicians in the documentation and completion of medical records.
- Evaluate facility records for completeness in accordance with established standards.
- Participate in quality assessment and improvement initiatives in line with facility policies.
- Review coding appropriateness upon request and respond promptly to third-party inquiries.
- Maintain registries for cancer and congenital malformations, ensuring compliance with reporting requirements.
- Attend relevant meetings, seminars, and workshops as necessary.
- Provide assistance with computer applications and system maintenance.
- Uphold confidentiality regarding medical record documentation in accordance with organizational policies.
- Perform additional duties as assigned while maintaining a clean and safe work environment.
- Preferred certification as a Certified Coding Specialist (CCS). Candidates with RHIA or RHIT eligibility may also be considered if enrolled in a relevant program.
- Certification as a Certified Professional Coder (CPC) or Certified Medical Coder (CMC) from the American Academy of Professional Coders (AAPC) is required.
- A degree in Health Information Management or related field is preferred.
- Successful completion of certification within one year of hire or graduation is expected.
- Candidates must pass a coding assessment.
- Maintain credentials through continuing education as required by AHIMA or AAPC.
- Preferred experience of six months in coding within an acute care setting.
- Thorough understanding of ICD-9-CM and CPT coding systems, along with medical terminology, anatomy, and physiology.
- Ability to collaborate effectively with healthcare teams and demonstrate strong communication skills.
- Proficient in utilizing technology and software relevant to coding tasks.
- Strong analytical and problem-solving skills.
- Ability to meet deadlines and generate necessary reports.
