The Forest at Duke is a premier retirement community featuring a state-of-the-art Health Center with a commitment to high-quality care. Our staffing ratios ensure personalized attention: 1 CNA to 5 residents & 1 Nurse to 20 residents.

The Nurse Care Manager plays a crucial role in delivering individualized, rehabilitative, and holistic care, focusing on chronic disease management for our residents. Our Health Center embraces the Small House Model of care, which empowers residents to have a significant voice in their daily lives, care, and living environment.

This position involves three 12-hour shifts, including every other weekend, with competitive compensation up to $34.75/hour for LPNs and $40.00/hour for RNs.

Available Positions:

Full Time - three 12-hour shifts: 7:00pm - 7:30am (Nights) & 7:00am - 7:30pm (Days)

Part Time - every other weekend: 7:00pm - 7:30am (Nights)

PRN - as needed: 7:00pm - 7:30am (Nights) & 7:00am - 7:30pm (Days)

Position Overview:

We are looking for a committed Care Manager to oversee and coordinate the healthcare needs of our residents, ensuring they receive exceptional care and support.

Key Responsibilities:
  • Utilize expertise in various medical fields to assess patient needs and develop tailored care plans.
  • Coordinate patient care activities, including scheduling appointments and monitoring treatment plans.
  • Collaborate with healthcare providers to ensure seamless service delivery and optimal patient outcomes.
  • Conduct utilization management to enhance patient care efficiency.
  • Provide education to patients on health management and preventive care strategies.
  • Maintain accurate patient records and documentation.


Experience Required:
  • Prior experience in nursing or a related healthcare field is essential.
  • Proficiency in patient care practices is required.
  • Familiarity with utilization management processes is advantageous.
