The Forest at Duke is an upscale retirement community that offers a state-of-the-art Health Center with exceptional staffing ratios of 1 CNA to 5 residents and 1 Nurse to 20 residents.
The Nurse Care Manager is responsible for directing and delivering person-centered, rehabilitative, holistic care and chronic disease management to residents. Our Health Center employs the Small House Model of care, emphasizing a person-centered approach that empowers residents in their daily lives, care, and living environment.
Position Details:
This role involves working three 12-hour shifts, including every other weekend, with competitive pay rates of up to $32.75/hour for LPNs and $38.00/hour for RNs.
Available Shifts:
- Full Time: 3/12-hour shifts from 7:00pm - 7:30am (Nights) or 7:00am - 7:30pm (Days)
- Part Time: Every other weekend from 7:00pm - 7:30am (Nights)
- PRN: As needed from 7:00pm - 7:30am (Nights) & 7:00am - 7:30pm (Days)

Key Responsibilities:
- Supervise the care team for assigned households, including staffing, performance evaluations, and training
- Provide direct care, medication administration, and treatments to meet resident needs
- Conduct clinical rounds on all assigned residents each shift
- Collaborate with physicians and the interdisciplinary team to ensure holistic wellness
- Manage resident medications in partnership with physicians and pharmacists
- Lead care planning and assessments, adjusting care plans as necessary
- Complete MDS assessments in EHR
- Perform other RN duties as assigned
Key Requirements:
- Must be a licensed nurse (RN or LPN) in North Carolina with 3-5 years of experience; geriatric nursing preferred
- At least two years of experience as a Charge Nurse or Nurse Supervisor required
- Strong computer skills; experience with Electronic Health Records preferred
- Certification in ServSafe and BLS/CPR required within the first 90 days of employment
- Knowledge of nursing and medical assessment practices and regulations governing long-term care
