Job Summary:
Liberty Health is seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals with terminal illnesses.
Responsibilities:
- Apply a working knowledge of nursing theories and concepts to provide comprehensive care to patients and their families.
- Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure seamless care delivery.
- Communicate effectively with patients, families, and healthcare team members to ensure clear understanding of care plans and progress.
- Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure high-quality care and patient satisfaction.
- Collaborate with the Hospice team to develop and implement individualized care plans that meet the unique needs of each patient.
Requirements:
- Current RN licensure in the state of practice.
- Successful completion of a nursing education from an approved school of nursing.
- At least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
- Working knowledge of the principles and practice of nursing, including reimbursement guidelines and agency policies and procedures.
- 12 hours of continuing education annually.
- CPR certification.
- Ability to accept travel assignments and work flexible hours, including weekends, evenings, and holidays.
