Job Title: Co-Occurring Disorders Case Manager

ReDiscover is seeking a skilled and compassionate Co-Occurring Disorders Case Manager to join our Integrated Treatment of Co-Occurring Disorders team. As a key member of our team, you will work closely with individuals who have both mental health and substance use disorders to provide support and guidance in achieving their recovery goals.

Key Responsibilities:
  • Maintain and interact with a caseload of clients with diverse characteristics, either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes of symptom management, employment, and independent living based upon the needs of the client.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, which may include helping clients participate in community or social service programs, referring clients to educational or working programs, and/or assisting with transportation barriers as identified in their treatment plan.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.
Requirements:
  • 4-year degree in a social science field or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver's license, appropriate automobile insurance, and a safe, reliable, registered automobile available for use during work.
  • At least two (2) years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.
  • Working knowledge of mental illness and substance use disorders.
  • Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, and Excel programs as well as GSuite.
