Job Summary

We are seeking a highly skilled and compassionate RN Case Manager to join our team at UnitedHealthcare. As a Field-Based RN Case Manager, you will play a critical role in coordinating care for our Medicare and Medicaid members, ensuring they receive the highest quality of care and support.

Key Responsibilities
  • Assess and develop individualized care plans for members, taking into account their unique needs and circumstances.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
  • Utilize strong communication and interpersonal skills to engage members in their care and empower them to take an active role in managing their health.
  • Provide education and support to members on disease management, medication adherence, and healthy lifestyle choices.
  • Identify and address social determinants of health, such as housing, food insecurity, and transportation barriers, to promote overall well-being.
  • Work closely with interdisciplinary teams to ensure timely and effective care transitions, reducing hospital readmissions and improving health outcomes.
  • Document and maintain accurate and comprehensive records of member interactions, care plans, and outcomes.
Requirements
  • Current, unrestricted RN license in the state of Kansas.
  • 2+ years of experience working in the community as a RN in acute inpatient care, long-term care, home health, hospice, public health, behavioral health, or assisted living.
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook.
  • Ability to travel up to 75% of the time with reliable transportation, with a current and non-restricted state of Kansas Driver's License and State-required insurance.
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI).
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.
Preferred Qualifications
  • Bachelor's degree in nursing.
  • A background in managing Medicaid and Medicare populations with complex medical and behavioral needs.
  • Experience with arranging community resources.
  • Experience with electronic charting.
