Job Summary

The Hub Clinical Care Coordinator plays a critical role in managing patient placement activities across the organization. This position serves as the primary liaison between nursing units, service locations, and support departments, ensuring effective and efficient patient-focused outcomes.

Key Responsibilities
  • Coordinate patient placement activities, including admissions, transfers, and discharges, to optimize system-wide patient flow.
  • Designate unit and bed assignments based on patient clinical needs, nurse staffing, and unit bed priority status.
  • Anticipate and manage barriers that may impede system-wide flow, escalating issues as necessary.
  • Evaluate and facilitate implementation of appropriate accommodations for patient care needs.
  • Manage patient placement, direct admits, and transfers in Care Logistics software.
  • Leverage information to adjust resources and facilitate patient flow efficiency.
  • Consult and educate medical and nursing staff regarding patient placement, payor requirements, and utilization of resources.
  • Manage the centralized transfer center and coordinate registration, insurance authorization, and care management clerical duties.
  • Partner with the Order Organizer and Staffing Coordinator to optimize system-wide patient flow.
Requirements
  • Minimum of three years recent acute care clinical experience.
  • Bachelor of Science in Nursing required.
  • Able to work independently, managing time, multiple priorities, and resources to achieve goals.
  • Able to assimilate information quickly to produce sound decisions and recognize situations that require immediate intervention.
  • Able to maintain a calm demeanor and command during time of crisis management.
  • Able to articulate information and ideas clearly through written and verbal communication.
Preferred Qualifications
  • One year supervisory experience strongly preferred.
  • Bed management experience strongly preferred.
  • Utilization review experience.
License and/or Certifications
  • Valid RN License from Virginia or reciprocal compact state required.
  • AHA BLS Provider CPR required at hire.
