Job Summary:

As a Hospice Nurse with Cantex Continuing Care Network, you will provide comprehensive nursing care to patients in their place of residence, coordinating care with the interdisciplinary team, patient, family, and referring agency. Your primary functions will include administering nursing care, coordinating care, and assuming responsibility for coordination of care.

Key qualifications for this role include a Registered Nurse license, 2 years of experience with a Hospice agency, and strong communication, assessment, and clinical documentation skills.

Responsibilities will include initial and ongoing comprehensive assessments, implementing and developing the plan of care, consulting with and educating patients and families, and providing clinical directions to Hospice Aides and LPNs/LVNs.
