Job Summary:

The Hub Clinical Care Coordinator is responsible for managing organizational-wide patient placement activities, ensuring effective and efficient patient-focused outcomes. This role serves as the leader of the system Hub, liaising between nursing units, service locations, support departments, and other healthcare facilities.

In partnership with physicians, the Hub C3 guides appropriate patient placement, collaborating with all system departments and healthcare teams to improve length of stay performance.

Key Responsibilities:
  1. Coordinate patient placement activities, managing 24/7 hospital-wide patient flow, including admissions, transfers, and discharges.
  2. Designate system-wide unit and bed assignments based on patient clinical needs, nurse staffing, and unit bed priority status.
  3. Anticipate and manage barriers to system-wide flow, escalating issues as necessary.
  4. Assess patients' clinical presentation against Milliman Care Guidelines to identify admission status, working DRG, and target LOS for direct admits and incoming facility transfers.
  5. Evaluate and facilitate implementation of appropriate accommodations for patient care needs.
  6. Manage patient placement, direct admits, and transfers in Care Logistics software.
  7. Leverage information to adjust resources, facilitating patient flow efficiency and achieving system-wide length of stay goals.
  8. Consult and educate medical and nursing staff regarding patient status, placement, payor requirements, and resource utilization.
  9. Manage the centralized, one-call transfer center for direct admits and facility transfers.
  10. Coordinate registration, insurance authorization, and care management clerical duties.
  11. Partner with the Order Organizer to identify and prioritize scheduling of ancillary and service department testing and procedures.
  12. Collaborate with the Staffing Coordinator to identify and prioritize nursing staff capacity.
  13. Serve as a resource on patient placement, system flow, and patient transfer management for the Hub, nursing unit, ancillary, and service department associates.
Qualifications:

Minimum of three years recent acute care clinical experience, Bachelor of Science in Nursing required. Able to work independently, managing time, multiple priorities, and resources to achieve goals.

Able to assimilate information quickly to produce sound decisions and recognize situations that require immediate intervention. Able to maintain a calm demeanor and command during crisis management.

Able to articulate information and ideas clearly through written and verbal communication.
