Responsibilities:

Position Summary:
This role is responsible for the comprehensive management and coordination of care for patients undergoing Kidney and Pancreas Transplantation. The level and intensity of care coordination will be tailored to meet the specific needs of each patient.

The areas of transplantation covered include:
pre-transplant assessment, waitlist management, post-transplant follow-up, and the timely allocation of organs. This position may necessitate Acute Care Nurse Practitioner certification such as ACNP or AG-ACNP for roles related to inpatient care.

Qualifications:

Bachelor of Science in Nursing is required.

Ability to establish and maintain effective working relationships across the organization is essential.

Must possess the skills, knowledge, and abilities necessary for successful performance in assigned duties.

Ability to maintain a work pace appropriate to the workload is required.

Demonstrated customer service skills suitable for the role are essential.

Skills to effectively perform leadership duties are required.

Excellent written and verbal communication skills in English are necessary.

Demonstrated knowledge of immunosuppressive therapies and post-transplant patient care is required.

Working knowledge of CMS/UNOS regulations is necessary.

A minimum of 2 years of experience in Critical Care, Step Down, or Dialysis is required.

A Master's Degree in Nursing or a related healthcare field, or 6 years of relevant experience is required.

A minimum of 3 years of experience in Transplant is required.

Specialty certification is required.

Acute Care NP certification such as ACNP or AGACNP is required for inpatient-related roles.

Preferred

Qualifications:


MASTER OF SCIENCE IN NURSING
Previous experience in transplant or organ procurement is preferred.

Licenses and Certifications:
RN is required.

NP is preferred.
