Job SummaryWe're seeking a highly skilled and empathetic Palliative Specialist to join our team. As a Palliative Specialist, you will provide comprehensive care to patients with chronic or serious illnesses, focusing on symptom management, pain relief, and improving the quality of life.
Key Responsibilities- Provide palliative care consultations to patients with chronic or serious illness.
- Develop and implement individualized care plans focusing on symptom management, pain relief, and emotional support.
- Collaborate with physicians, nurses, social workers, and other healthcare providers to ensure comprehensive, team-based care.
- Educate patients and their families about illness progression, treatment options, and goals of care.
- Monitor patient outcomes and adjust care plans as necessary.
- Current, valid certification as a Physician Assistant (PA) or Nurse Practitioner (NP) with specialization in palliative care, geriatrics, or related field.
- Certification such as Certified Hospice and Palliative Nurse (CHPN) is preferred.
- Minimum of 1-3 years of experience in palliative care, hospice, or chronic disease management.
- Strong communication skills with the ability to provide compassionate care and counseling to patients and families.
- Ability to work independently, manage a diverse caseload, and coordinate care in a remote setting.
