Key Responsibilities:
  • Provide information, outreach, and assistance to consumers and the public at large, collecting information, determining service eligibility, and accepting referrals for services as assigned.
  • Maintain familiarity with and inform persons served of their rights and responsibilities, ensuring their rights are protected during services delivery.
  • Demonstrate engagement and responsiveness with consumers and their support teams, assessing current functioning and priorities, and determining the type, times, and range of services or supports required.
  • Develop comprehensive, outcome-focused service plans in response to needs, securing, arranging, or appropriating the resources necessary to meet them.
  • Provide, coordinate, regularly monitor, and/or adjust services as planned, ensuring their ongoing focus, effectiveness, and efficiency.
  • Maintain case records, including timely and accurate documentation of activity, progress toward goals, and changes in consumer status or plans.
  • Analyze the need for court orders or intervention, prepare court reports, assist with court appearances, and provide testimony as may be required.
  • Maintain awareness of and compliance with applicable rules, policies, procedures, funding source requirements, professional standards, and evidence-based practices as may apply.
  • Participate in unit and department-wide meetings, discussions, training, and activities, demonstrating orientation toward professional competence and continuous quality improvement, making recommendations for and assisting with service improvement when warranted.
Requirements:
  • A minimum of a bachelor's degree in behavioral science and 1,000 hours of supervised post-degree clinical experience serving individuals with severe and persistent mental illness, or a bachelor's degree in a field other than behavioral science with 2,000 hours of supervised post-degree clinical experience serving persons with severe and persistent mental illness preferred.
  • Knowledge of or experience with trauma-informed principles and practices is preferred.
  • Knowledge and understanding of federal laws, State statutes, and administrative rules which regulate the services to be provided.
  • Knowledge and understanding of the programs, funds, and services available.
  • Knowledge of and sensitivity to social problems, cultural diversity, human growth and development, and individual and family functioning.
  • Ability to coordinate activities and functions that cross unit and departmental lines and involve other county and community agencies.
  • Ability to establish and maintain effective working relationships with co-workers, superiors, community partners, and the public at large.
  • Ability to relate to people and to communicate effectively in both written and oral formats.
  • Ability to maintain confidentiality.
  • Ability to exercise sound judgment in confrontational and stressful situations.
  • Ability to follow instructions, to complete tasks, and to work closely with the supervisor and others.
  • Must have unlimited access to a car, a valid driver's license, automobile insurance, and the ability to efficiently travel in and out of county, as needed.
Work Environment & Physical Requirements:
  • Frequent contact with subordinates, co-workers, consumers, State and County personnel, other authorities, community partners, and members of the public.
  • May occasionally deal with aggressive or violent individuals.
  • Work is performed in a variety of home, community-based, and office settings and involves travel. Ability to lift up to 20 pounds and a combination of sitting and standing postures may be required. Ability to minimally work 8 hours per day and 40 hours per week is required.
  • Must be able to operate and be conversant with office equipment, including phone, computer, facsimile, calculator, printer, and photocopier. Familiarity and capacity to work with various software applications and keyboarding proficiency are essential.
