Advanced Practice Provider

The Advanced Practice Provider at Prevea Health is responsible for delivering healthcare services to patients, families, and groups, focusing on health promotion and disease prevention under the supervision of physicians.
Key Responsibilities- Provides health assessment, evaluation, and management for patients with minor, acute, chronic, or episodic illnesses and injuries.
- Collaborates with patients to review their health background, determine their physiological, emotional, and environmental needs, and advise them on future health plans and illness prevention.
- Provides case management services to assist clients and families in arranging for or referring patients to needed services.
- Conducts telephone calls to patients requiring follow-up care or provides test results, and offers health counseling and education to patients and their families about their healthcare and any follow-up instructions.
- Maintains and reviews patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
- Collaborates with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes for patients.
Master's level nurse practitioner program or graduate from an accredited physician assistant program.
Experience Qualifications1-3 years of work experience in a clinical setting within a healthcare organization.
Skills and Abilities- Knowledge of information and techniques required to diagnose and treat human injuries and diseases.
- Organizational skills to manage patient appointments and follow-up care.
- Excellent interpersonal skills and ability to form work-patient relationships.
- Ability to prioritize work and meet changing demands.
- Ability to handle and process confidential information.
- Fosters teamwork through effective written and verbal communication.
Nurse Practitioner: Registered nurse in the state of Wisconsin with a certification through American Nurses Credentialing Center (ANCC) or American Nurse Association (ANA).
Physician Assistant: Licensed by the state of Wisconsin as a Physician Assistant, with certification from the National Commission on Certification of Physician Assistant, and CPR/Basic Life Support for Healthcare Providers certification from an approved program.
