Qualifications

Education:

  • Graduation from an accredited medical school is required.
  • Completion of an accredited residency program is mandatory.
  • For specialty physicians, completion of an accredited fellowship in the relevant specialty is necessary.

Licensure/Certification/Registration:

  • Possession of a current MD license in the relevant state.
  • Board certification or eligibility is required.
  • A current DEA license is mandatory.
