Skilled Nursing Care Provider

About the Role:

We are seeking a highly skilled and compassionate Skilled Nursing Care Provider to join our team at NYC Health Hospitals/Community Care. As a key member of our Community Care team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.

Job Description:

The ideal candidate will have a minimum of 2 years of full-time nursing experience, with at least 2 years of experience in Homecare, Long-term Care, or Hospital settings. You will work closely with patients, families, and caregivers to assess, plan, implement, and evaluate individualized care plans that promote optimal health outcomes.

Responsibilities:

  • Assess patients' physical, social, and emotional needs, developing comprehensive care plans that address these needs.
  • Provide direct skilled nursing care, including wound care, medication management, and education on disease management.
  • Coordinate services with other healthcare professionals, including physicians, therapists, and home health aides.
  • Maintain accurate and complete records, documenting patient progress and care provided.
  • Participate in case conferences and interdisciplinary team meetings to ensure collaborative care planning.

Requirements:

  • Licensed and currently registered to practice as a professional nurse in New York State.
  • Current certification in Basic Life Support procedures, including CPR.
  • Baccalaureate Degree in Nursing from an approved program (preferred).
  • Demonstrated knowledge of Federal, State, and local regulations and JCAHO standards for Home Care.
  • Excellent verbal, written, organizational, and analytical skills.

Compensation:

The estimated salary for this position is $65,000 - $85,000 per year, depending on experience and qualifications.
