We are currently seeking a Patient Care Coordinator Leader to oversee the coordination of patient care activities, manage staff, and coordinate with support departments.

Job Summary

This position is responsible for supervising staff, managing inpatient admissions, reviewing KPIs, training and monitoring staff performance, preparing schedules, liaising with support departments, ensuring training compliance, validating EMR revisions, and tracking performance metrics via Epic reports.

Key Responsibilities
  • Supervise Tour III/I staff, manage inpatient admissions, review KPIs, train and monitor staff, prepare schedules, liaise with support departments, use PeopleSoft for performance assessments, ensure training compliance, validate EMR revisions, and track performance metrics via Epic reports.
  • Implement process improvements to enhance patient care and satisfaction.
  • Develop and maintain positive relationships with patients, families, and healthcare providers.
Requirements

Candidates must have a Baccalaureate Degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology, or Rehabilitation Counseling, and three years of full-time experience in medically-oriented health care and medical support systems environments, two years of which included experience in an administrative or supervisory capacity.
